Instructions for ESC Request Form
Fill out each of the gray areas on the form. These areas include:
- Date of Request (The form contains drop-down boxes for the month and year)
- Passport Nationality
- Passport Number
- Full Name of Traveler (example: John SMITH)
- Official Title
- Date of Birth (The form contains a drop-down box for the month)
- Country of Birth
- City of Birth
- Point of Contact
- Organization
- Telephone and Fax Numbers
- After Hours Telephone Number(s)
- E-Mail Address for Confirmation
- Airline and Flight No.
- Departure Airport
- Month/Day/Year (The form contains drop-down boxes for the month and year)
- Time (the appropriate box should be checked for AM or PM)
- Itinerary Continues on Next Page (“yes” or “no”)
The ESC form will be returned if any of the aforementioned critical fields are left blank.
